Membership Benefits:

As a member, you’re entitled to the benefits associated with your chosen membership. These benefits are valid for the duration of your membership (12 month commitment) and are subject to change without prior notice.

Payment and Billing:

Membership fees are charged on a monthly basis and are not payable in advance. By becoming a member, you authorize Pure 111 to collect payment each month. It is the patient’s responsibility to ensure their payment information is accurate.

Cancellation Policy:

You reserve the right to cancel your membership after your 12 month commitment. There are no hidden cancellation fees and you can rejoin at any time. However, we recommend first redeeming any unused benefits or services in your wallet, as these will no longer be valid upon cancellation.

Service Availability:

Membership benefits and discounts apply to the eligible service offered by Pure 111. Please note that this service may have specific eligibility requirements or restrictions. Our team will provide you with details regarding the applicability of benefits to this service. Migraine Membership may only be used on the applicable service.

Non- Transferability:

Memberships and their associated benefits are non-transferable. They are solely for the use of the individual member and cannot be shared, assigned, or transferred to another person.

Modification of Terms:

Pure 111 reserves the right to modify or update these terms without prior notice. You can review the membership terms periodically for changes. Continued use of the membership program constitutes acceptance of any modifications made.

Do you accept insurance?

We do not accept insurance; however, we do accept HSA/ FSA and care credit.

Can I Pause My Membership?

While we understand the need for flexibility, we currently do not offer an option to pause or temporarily suspend memberships. However, you have the freedom to cancel your membership after the initial 12 month commitment and rejoin when you’re ready.

Can I cancel at Anytime?

The membership operates on an annual renewal basis. If you decide to not renew, you will not be charged any fees. You may rejoin our membership at any time.

When Can I Schedule My Treatment?

Once you become a member, you can schedule your treatments at a time that is convenient for you. Our helpful staff will assist you in finding the best appointment slots.

Where Can I Purchase a Membership?

To become a member, talk to your provider or one of our patient coordinators. Our helpful team will guide you through the sign-up process and answer any questions you may have.

Do I Receive Any Benefits for Referring a New Member/Patient?

We appreciate your referrals! To show you appreciation, we offer a refer a friend program! When you refer someone as a client, you will receive $25 off and the individual you referred will also receive $25 off their purchases.
